1:1 Disability Support for NDIS Participants

 

Shape Your Support is the leading provider of male-specialised disability support in the Geelong Region. Our aim is to help our clients build life and social skills through transforming everyday activities into active teaching situations and opportunities for practice. Our 1:1 supports are aimed at supporting our clients in accessing the community to help them meet their goals.

We work together with families, care professionals and other supports to ensure that our clients are getting the most out of their support and making steady progress towards their individual goals.

As we specialise in male-to-male disability support we are incredibly passionate about breaking down concepts of toxic masculinity and providing positive male role models who are dedicated to forming long-lasting and meaningful relationships with those they work with.

 

The Intake Process

 

At Shape Your Support, finding the right match is very important to us. We don’t just fill shifts — we aim to build meaningful, impactful relationships between our participants and their support workers. The key to this is our intake process, which is designed to help us understand each person as the human they are, not just their NDIS number.

  • Age of participant

Intake Form

We know how exhausting it can be filling out countless forms when engaging with disability support services, so we have designed our Intake Form to ensure that we get all of the information we need in one go. Our Intake Form provides us with everything we need to know to ensure we have an adequate understanding of the participant, their needs and whether we can accomodate them before moving forward and pairing them with a support worker.

 

Meet and Greets

Finding the right match is important to us! Before commencing support with a new family we always arrange for a Meet and Greet to ensure that both support worker and client have the opportunity to meet each other and see if they are the right fit. Meet and Greets are complimentary and will always be somewhere that is comfortable for our client and their family.
